Connect to the ICN

Connecting to the ICN is easy. Download the application form and mail or fax it to us. All forms are in Adobe Acrobat format.

 For new ICN applicants:

Download a copy of the application, fill it out, and fax or mail it to us. Our fax number and mailing address are listed on the application form. This form is required of all buildings connecting to the ICN either directly or indirectly. If a building houses multiple entities from the same organization or from the different organizations, each entity must submit a completed ICN Participation Application.

ICN Participation Agreement
This is the agreement form to participate in the ICN. The ICN Participation Agreement is required of all fiscal organizations whether connecting directly or indirectly to the ICN.

The ICN Service Agreement will be completed by your Regional Technology Center and sent to constituents who are directly connected to the ICN as a follow up to their Participation Agreement. The form is provided here for information purposes and does not need to be filled out or returned.

Acceptable Use Policy

Additional IP Addresses Request
If you are a current Participant and require additional IP addresses because of growth in computers, network arrangement, or Internet services, please fill out and return this form.

Additional Buildings/Organizations Request
This form is used when connecting additional buildings and organizations to an existing ICN member.

Cost Recovery Information
The ICN Policy Committee has approved a cost recovery model to help fund the future growth of the Illinois Century Network.

E-rate LOA
Form needed from all e-rate eligible entities acknowledging that the ICN is filing for reimbursement on the constituents behalf.

ICN Management Release Agreement
This waiver form is needed if you want to maintain and troubleshoot your own router.

Letter of Agency
This document is needed in certain situations and allows the ICN to order circuits on behalf of the customer.

Welcome Packet
New participants in the ICN are sent a welcome packet that includes a summary of the ICN, support numbers and other information related to their connection.
