| ICN E-rate Eligible
Constituents,
Since the inception of the e-rate program, the Illinois Century Network (and its predecessor the LincOn Network) has received e-rate funding as a consortia based on the networks expenditures to provide services to e-rate eligible participants. In order to continue to receive these valuable funds, the ICN needs to secure a Letter of Agency (LOA) from all e-rate eligible entities acknowledging that the ICN is filing for reimbursement on the constituents behalf. For your convenience, we have included a Frequently Asked Questions document that provides more information on why the LOA is needed and how it impacts your organization. If you have any additional questions or concerns that are not addressed in the FAQ, please feel free to call your ICN Regional Technology Center. Be assured that the ICN does not require constituents to comply with the e-rate rules in order to participate in the Network. If you do not comply with the e-rate rules, simply check the appropriate box on page two of the LOA, sign and date the form, and return to the ICN. However, the Network does receive significant funding from the e-rate program and therefore encourages its constituents to comply with the rules if at all possible. Please complete the LOA, sign and date the form, and return an original signature to the ICN by June 28, 2002. Completed forms should be mailed to ICN E-rate LOA, 120 W Jefferson, Suite B, Springfield IL 62702-5172. If you have any questions or concerns, please contact your ICN Regional Technology Center or feel free to contact me at 217.557.6565. Thank you for your prompt attention to this request.
